Transforming Patient Care

Care Management activities are conducted through a combination of telephonic and face-to-face interactions, including visits to member homes, in the community, and/or provider locations. The goal is to provide quality care, ensure compliance, and improve outcomes while reducing costs.

Key Responsibilities

Participate in the care planning process in collaboration with the Care Manager

Conduct initial and periodic assessments of members enrolled in the Long-Term Care Program and/or care programs

Develop and carry out strategies to coordinate and integrate post-acute and long-term care services to members to prevent exacerbations and/or placement of the members in custodial care

Apply social work concepts, principles, and strategies in addressing the social determinants of health needs in members' individualized care plans

Requirements

Active and unrestricted LSW or LCSW licensure in the state of Pennsylvania

Minimum of 2+ years experience in medical social work or case management

Ability to travel 50-75% within the designated geographic area with reliable transportation and mileage reimbursement
