About the Role
Graham Healthcare Group is seeking a skilled Caregiver Support Specialist to join our team. As a Journey Counselor, you will provide essential information, education, and support to patients, families, and caregivers regarding end-of-life planning and care.
Your Key Responsibilities
- Manage patient census and referrals for assigned areas, ensuring timely transitions to Hospice care.
- Review patient charts to determine eligibility and appropriateness for Hospice services, preparing for conversations with patients, families, and caregivers.
- Conduct initial phone calls to new referrals, scheduling home visits and maintaining contact with patients to assess their needs.
- Build relationships with patients and their families, providing in-depth discussions about the Hospice benefit when appropriate.
- Maintain accurate records of patient interactions, documenting all visits and contacts in patient charts.
Requirements
- High school diploma or equivalent required; bachelor's degree preferred.
- Prior experience in home healthcare or a related field is beneficial.
- A strong understanding of the hospice philosophy is advantageous.
