Job Title: Registered Nurse

Job Summary:

We are seeking a skilled and compassionate Registered Nurse to join our team at Freedom Home Health And Hospice Care Services Inc. As a Registered Nurse, you will play a vital role in providing high-quality patient care and education to individuals and families in their homes and communities.

Key Responsibilities:

  • Conduct initial assessments of patients and families to determine home care needs and develop personalized care plans.
  • Provide comprehensive physical assessments and histories of current and previous illnesses.
  • Regularly re-evaluate patient nursing needs and make necessary revisions to care plans.
  • Initiate preventive and rehabilitative nursing procedures, administer medications and treatments as prescribed by physicians, and counsel patients and families on meeting nursing and related needs.
  • Provide health care instructions to patients as appropriate per assessment and plan of care.
  • Identify discharge planning needs and implement prior to discharge of patients.
  • Act as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and coordinate patient care for assigned caseload.

Requirements:

  • Graduate of an accredited school of nursing with one to two years of recent acute care experience in an institutional setting.
  • Current licensure in state and CPR certification.
  • Bachelor's degree with one year of home health care experience preferred.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
  • Prolonged or considerable walking or standing, ability to lift, position, or transfer patients, and ability to lift supplies and equipment.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
