Job Summary

We are seeking a skilled and compassionate Pediatric Care Coordinator to join our team at Children's Healthcare of Atlanta. As a key member of our care coordination team, you will play a vital role in ensuring seamless transitions for our patients and families.

Key Responsibilities
  • Conduct thorough assessments of patients to identify care coordination needs and develop individualized transition plans.
  • Cultivate strong relationships with multidisciplinary teams to facilitate effective communication and collaboration.
  • Coordinate with home health agencies, third-party payors, and community resources to ensure timely and efficient discharge planning.
  • Provide ongoing support and education to patients and families to empower them to manage their care and make informed decisions.
  • Participate in quality improvement initiatives to drive best practices and optimize patient outcomes.
Requirements
  • 3 years of experience in pediatric nursing with a Bachelor of Science in Nursing (BSN) or current Children's nurse with 5 years RN experience with an Associates degree in Nursing.
  • Current licensure as a Registered Nurse in the state of Georgia or Multi-State through the Enhanced Nurse Licensure Compact.
  • Registered Nurse case management certification from an approved accrediting organization within 1 year of meeting eligibility requirements.
Preferred Qualifications
  • Master's degree in nursing or related field.
  • Experience in care coordination, case management, discharge planning, and utilization review.
