Job Summary:

As a skilled Occupational Therapist, you will provide high-quality patient care services to individuals in their place of residence, addressing impairments, cognitive, and physical limitations resulting from injury, disease, or other causes. Collaborate with patients, families, physicians, and other healthcare professionals to identify patient needs and coordinate services to meet those needs. Manage technical, environmental, and financial resources effectively.

Key Responsibilities:

Develop and implement patient-centered plans of care, incorporating standardized interventions for goal-oriented outcomes.

Ensure accurate and timely documentation in our computerized system, reflecting objectivity and measurability.

Apply evidence-based treatment procedures, providing education to patients and caregivers.

Maintain regular communication with interdisciplinary teams.

Conduct comprehensive clinical evaluations, incorporating patient history, relevant systems review, and appropriate tests and measures.

Reassess and modify plans of treatment at least every 2 weeks, ensuring alignment with discharge goals.

Verify that physician orders are current and consistent with the plan of care, documented in the medical record.

Submit patient schedules daily.

Supervise Home Health aides according to agency and state guidelines.

Optimize capacity and ensure compliance with visit schedules by utilizing OTAs.

Manage patient caseloads of Occupational Therapy Assistants, adhering to agency, state, and professional guidelines.

Complete and submit documentation in a timely manner, meeting agency standards.

Update POC devices as directed.

Verify Payor information on admission and attend to it according to agency standards.

Maintain and transmit documentation to support Managed Care authorizations/DME requirements within designated time frames.

Ensure visit schedules align with authorizations by Managed Care payors.

Identify realistic, objective, time-oriented goals for patient performance based on examination findings.

Discuss utilization/caseload with Manager as needed and make changes toward stated goals.

Ensure reportable outcomes meet Agency goals.

Select treatment and discharge plans based on examination findings and patient goals.

Attend staff meetings and education in-service regularly, completing clinical competencies and mandatory training as required.

Inform patients of their weekly schedule and any changes as necessary.

Required Qualifications:

Registration with the Board of Registration in Massachusetts.

Experience:

Two years of experience in acute care, rehab, or homecare as an Occupational Therapist.

Ability to work with various computer software required and knowledge of hand-held patient care computer devices preferred.

Strong customer service skills and ability to work with diverse populations.

Excellent verbal, written, and electronic communication skills.

Ability to prioritize work and adapt to changing situations.

Travel required within PHH's geographic area.

Certifications:

Must be a Certified Hand Therapist.
