Job Summary

We are seeking a skilled Physical Therapist to join our team. As a Physical Therapist at Ambercare, you will provide physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.

Responsibilities
  • Provide Physical Therapy Services: Deliver high-quality physical therapy services to patients, including advising, consulting, and teaching treatment procedures and supportive activities.
  • Evaluate Patient Function: Assist the provider in evaluating the patient's level of function and developing a plan of care.
  • Prepare Clinical Notes: Prepare clinical and progress notes, translating exercises into functional activities or activities of daily living.
  • Modify Equipment and Environment: Develop plans for modifying equipment, appliances, and the physical surrounding in clients' homes.
  • Communicate with Providers: Confer regularly with attending providers regarding client progress and report physical and emotional conditions and reactions to treatment.
  • Support Patient Families: Provide families with information, support, and encouragement to help motivate them in their progress.
Qualifications
  • Graduate of a School of Physical Therapy: Must be a graduate of a School of Physical Therapy approved by the American Physical Therapy Association or Allied Health Education and Accreditation of the American Medical Association and the Physical Therapy Association.
  • Current Licensure: Must have current licensure as a Physical Therapist in the state of practice.
  • Experience and Certification: At least two (2) years' experience in an acute hospital or equivalent experience (preferred), with current CPR and First Aid Certification.
  • Reliable Transportation: Must have reliable transportation, a current driver's license, and appropriate automobile insurance.
