Position Overview

Moms In Motion is seeking a dedicated SFC Courier Case Manager to enhance our support for families in the Westmoreland, Caroline, Essex, and King George areas.

The SFC Courier will collaborate closely with the SF Data Clerk to ensure seamless service delivery to our clients. This role involves visiting clients' residences to review essential documentation, secure necessary signatures, and upload information for processing, thereby preventing any interruptions in client services.

Key Responsibilities:

  • Engage with clients in their homes to facilitate documentation processes.
  • Maintain a flexible work schedule to accommodate client needs.
  • Utilize a reliable vehicle for transportation to client locations.
  • Employ a portable device with internet access and signature capture capabilities.
  • Possess a minimum of an Associate's Degree in Human Services or a related field, coupled with 2 years of relevant experience; or a Bachelor's Degree in any discipline with 3 years of pertinent experience.
  • Exhibit strong organizational skills, proficiency in scheduling, and dependability, with a keen focus on documentation accuracy.
  • Demonstrate a documented blend of skills and relevant work experience. Training in Person-Centered Thinking/Planning is advantageous.
  • Familiarity with Medicaid waivers is a plus.
