Job Summary and Qualifications

The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse, collaborating with nursing staff in the planning, implementation, and evaluation of nursing care. LPNs are responsible for observing, monitoring, and reporting patient symptoms, reactions, and progress to the healthcare team.

Key Responsibilities:

  • Monitor, document, report, and follow up on symptoms or changes in patients' conditions, notifying the RN as necessary.
  • Create and maintain accurate, detailed reports and records.
  • Collaborate with healthcare team members to assess, plan, implement, and evaluate the care plan based on patient responses and conditions.
  • Perform nursing tasks safely and appropriately under the indirect supervision of the RN.
  • Provide individualized education for patients, families, and caregivers throughout the continuum of care as directed by the RN.
  • Adhere to safety and infection control standards in delivering patient care.
  • Demonstrate thorough knowledge of and comply with all policies and procedures related to job duties.
  • Perform additional duties and special projects as assigned by the RN, Nurse Manager, or Director.

Qualifications:

  • Strong interpersonal skills to effectively interact with internal and external customers.
  • Graduate of an accredited School of Practical Nursing.
  • Current licensure by the KY Board of Nursing.
  • Current BLS Certification.
  • Some specific departments may require completion and graduation from an accredited School of Nursing with an ADN within 3 years or BSN within 4 years from the date of hire.
