Job SummaryWe are seeking an experienced Home Hospice Aide to join our team at Grand View Health. This role requires a compassionate and skilled individual who can provide high-quality patient care in a home setting. As a Home Hospice Aide, you will work under the direction of a professional nurse to deliver exceptional care to patients and their families.
Key Responsibilities:
- Provide personal care to patients at their home, including assistance with daily activities and incidental duties.
- Assess patients' physical condition, including temperature, pulse, respirations, and blood pressure.
- Support patients with prescribed exercises and promote overall well-being.
- Observe patients' condition and report any changes to the Registered Nurse or Supervisor.
- Document care provided and other pertinent observations.
- Complete patient assignments within acceptable timeframes.
- Deliver empathetic care to patients and their families during challenging times.
To be successful as a Home Hospice Aide, you must possess:
- A High School Diploma or equivalent.
- Basic Life Support (BLS) certification within two weeks of hire.
- A valid PA driver's license and auto insurance.
- Certified Nursing Assistant (CNA) or two years of experience in hospital, extended care facility, home care, or Hospice after completion of 75 hours of supervised practical training.
- Nursing students completing one academic year inclusive of direct patient care may be considered if certification requirements are fulfilled.
