Key Responsibilities
Position Overview
The Physical Therapist will assess all assigned patients, crafting, executing, and adjusting treatment plans tailored to each individual's needs and physician recommendations. This role includes overseeing and guiding all therapy services specific to the discipline for those patients. The therapist will design functional treatment programs that facilitate a successful return to community activities. Additionally, the therapist will serve as an educator for patients, families, students, and colleagues, while performing other professional duties related to patient care and therapy service development.
Specific Responsibilities
- Assess assigned patients and set appropriate treatment objectives and programs.
- Continuously evaluate patient progress and adjust treatment plans to ensure timely advancements.
- Formulate comprehensive treatment goals to optimize patient outcomes and facilitate reintegration into community activities.
- Collaborate with other clinical and therapeutic services, as well as community resources, to coordinate treatment programs. Participate in relevant clinical meetings to enhance coordination.
- Provide patient education, including the creation of comprehensive home programs with guidance for patients, families, and caregivers to ensure safe and effective discharges.
- Prepare for patient discharges by ordering necessary equipment and recommending therapeutic services to maintain continuity of care.
- Assess patients in real or simulated home, school, work, or community settings as needed to support their return to community activities, offering recommendations for necessary adaptations or training.
- Identify and manage varying levels of care essential for effective care plan execution. Supervise and delegate tasks to PTAs and support staff to enhance patient outcomes and optimize service utilization.
Qualifications
Essential Qualifications
- Current state licensure or eligibility for state licensure as a Physical Therapist is required.
- Certification in BLS or completion within the first 90 days of employment is mandatory.
- Home Care Requirements: Minimum of one year of clinical experience and a valid state Driver's License.
