Job Summary

We are seeking a highly skilled Physical Therapist to join our team at a skilled nursing facility in Monticello, MN. As a Physical Therapist with Radius Staffing Solutions, you will have the opportunity to work with a dynamic and supportive team to provide high-quality patient care.

Responsibilities
  • Perform and document initial evaluations and ongoing assessments of patients' conditions, adapting treatment plans accordingly.
  • Develop and implement individualized plans of care to meet required frequency of visits and include patient/family education.
  • Communicate patient progress with patients, families, and interdisciplinary teams.
  • Supervise activities of assistants, students, and support personnel as needed.
  • Plan and assist in patient discharge.
  • Ability to work in a typical clinical environment, stamina to remain standing or sitting for extended periods, and lift/assist patients as needed.
Requirements
  • Active Minnesota Physical Therapist license or eligibility to obtain.
