Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Nampa West of Cascadia, LLC. As an LPN, you will provide high-quality patient care and work collaboratively with our healthcare team to ensure the best possible outcomes for our residents.

Key Responsibilities
  • Provide direct care and assist with care as directed and consistent with your scope of practice and competency.
  • Initiate the SBAR process and collaborate with the RN for assessment and recommendations.
  • Make rounds with physicians and other team members, as needed.
  • Participate in resident evaluations and care planning activities, reviewing and revising residents' assessments and care plans as needed.
  • Coordinate residents' care activities and communicate changes to other team members and resident family/significant other.
  • Administer medications as ordered, including next scheduled dose following admission/readmission.
  • Notify Charge Nurse of medications not available.
  • Prepare, administer, and document medications as prescribed.
  • Observe and report resident responses to medication.
  • Identify and promptly communicate adverse drug reactions.
  • Maintain narcotic records accurately within scope of practice.
  • Complete medication interchange and insurance prior to authorization as assigned.
  • Order or arrange for ordering of pharmaceuticals.
  • Notify physicians of automatic stop orders.
  • Complete recapitulated physician orders review, as assigned.
  • Clean, organize, and replenish supplies on medication/treatment carts after each use and prior to shift change.
  • Complete clinical admission paperwork for new admissions.
  • Initiate and assist consultants while providing services.
  • Review documentation from consultant visit and follow up on recommendation.
  • Assist with follow up on results of qualitative and quantitative medical record audits.
  • Communicate and implement pharmacy review/drug regime review recommendations, as assigned.
  • Assist with clinical discharge process.
  • Ensure punctuality and regular attendance for assigned shifts.
Requirements
  • Graduate of accredited LPN School of Nursing.
  • Current valid LPN license in the state employed.
  • Valid CPR certification.
  • Six months experience in a long-term care environment preferred.
Knowledge/Skills/Abilities
  • Ability to react decisively and quickly in emergency situations.
  • Ability to communicate effectively with residents and their family members, and facility staff.
