Job Summary

We are seeking an experienced Occupational Therapist to join our team in providing home health care services to our patients. As an Occupational Therapist with Elara Caring, you will have the opportunity to work with a diverse range of patients and make a real difference in their lives.

Key Responsibilities
  • Evaluate patients' occupational therapy needs and develop personalized plans of care to help them achieve their goals.
  • Implement and coordinate occupational therapy services to ensure high-quality care and continuity of care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Conduct thorough assessments and evaluations to identify patients' strengths and areas for improvement.
  • Develop and implement individualized plans of care to address patients' occupational therapy needs.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Document patient progress and outcomes to ensure accurate and timely reporting.
Requirements
  • Graduate of an accredited Occupational Therapy program.
  • Current state license as an Occupational Therapist.
  • 1+ year experience as an occupational therapist in a clinical care setting.
  • Home Health experience is preferred.
  • Effective verbal and written communication skills.
  • Ability to travel to meet patient needs within branch coverage area.
