The Department of Mental Health (DMH) Tewksbury Hospital Hathorne Units is seeking a skilled and compassionate Clinical Social Worker to enhance the level of service provided to clients within an inpatient hospital setting.
Key Responsibilities:
- Formulate independent psychosocial assessments, gathering information from current and past records, patient, family, significant others, and community providers as available.
- Establish timely linkages with community agencies to maximize continuity of care and timely discharges.
- Communicate with the Patient Care Reimbursement Investigators to ensure all entitlements are in place as soon as possible post admission, and in preparation for discharge.
- Ensure evaluations of ability to manage funds are complete as per DMH policy.
- Meet all Department of Mental Health, Department of Clinical Social Work, Hathorne Units, Joint Commission, Centers for Medicare and Medicaid Services (CMS) / Health Care Finance Administration (HCFA) requirements for documentation.
Requirements:
- Current and valid licensure as a Licensed Certified Social Worker or Licensed Independent Clinical Social Worker by the Massachusetts Board of Registration in Social Work.
- Proficiency in Microsoft Office and Electronic Medical Record systems.
- Bilingual or multilingual fluency.
