Position Overview:

Work Schedule:
Full-Time / Day Shift / 64 scheduled hours per pay period.


About Us:
Ministry Support (Diakon Lutheran Social Ministries) is dedicated to providing exceptional care in a picturesque senior living community. Our facility has a long-standing reputation for excellence in service and care.


Key Responsibilities:
- Deliver individualized nursing care in accordance with each resident's care plan.
- Assess and monitor residents' health conditions, collecting and analyzing relevant data to aid in care plan development.
- Maintain accurate electronic health records, documenting observations in compliance with federal and state regulations.


Qualifications:
- Must possess a current State license to practice as a Licensed Practical Nurse.
- Preferred: A minimum of one (1) year of experience in an acute care, rehabilitation, or skilled nursing environment.


Physical Requirements:
- Ability to perform physical tasks including sitting, standing, lifting, bending, reaching, pushing, and pulling items weighing up to 50 lbs. Visual and auditory skills are essential.
