Job Summary

Boundless Care, Inc. is seeking a skilled Certified Nursing Assistant to provide personal care services to senior participants receiving home care and/or day center services. As a Caregiver 2, you will work under the direction and supervision of the Supervisor/Manager to deliver high-quality care and support to our clients.

Responsibilities
  • Personal Care Services: Provide personal care and escort assistance according to the individual care plan, including grooming, dressing, pericare, showers, bathing, toileting, housekeeping, meal preparation/feeding, and laundry.
  • Reporting and Communication: Report observed changes in the participant's condition to the interdisciplinary team and maintain open communication with participants, families, and the team.
  • Professionalism and Safety: Demonstrate safe practices by utilizing proper body mechanics, adhering to universal precautions, and maintaining environmental safety.
  • Additional Duties: Perform other related duties as assigned by the Supervisor/Manager.
Requirements and Qualifications
  • Independence and Teamwork: Ability to work independently and as part of an interdisciplinary team.
  • Flexibility and Adaptability: Flexible to work in multiple environments, including home or center settings.
  • Positive Attitude and Skills: Positive can-do attitude, conscientious, compassionate, and thorough, with basic conversational English skills required.
  • Language Skills: Basic conversational English skills required; testing may be required. Bilingual may be required with ability to speak primary language to care for participant population; language testing may be required.
  • Documentation and Communication: Able to document care-related items in Basic English on designated forms or electronic health record.
  • Physical Requirements: Ability to stoop, reach, stand, walk, push, pull, lift, grasp, talk, hear, and repeat motions of the wrist and hands. Must have visual acuity and be able to lift/move weight within individual physical ability, using appropriate lifting devices or requesting assistance when needed.
