Hospice Social Worker

We are seeking a highly skilled and compassionate Hospice Social Worker to join our team in Santa Fe. As a key member of our interdisciplinary team, you will be responsible for performing psychosocial assessments of patients and their families/caregivers, ensuring identified needs are addressed in the patient's plan of care. You will also provide counseling and support to patients, families, and staff as needed, assist with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver, and obtain resources from community social, health, pharmaceutical, and welfare agencies to meet patient/family needs.

• Social Work Degree
• New Mexico: Master's Degree in Social Work required
• Current State Licensure in Social Work
• New Mexico: LMSW or LCSW
• Minimum of one year's experience in health care or hospice setting
• Valid driver's license and proof of insurance
