Your Role

We're seeking an experienced Registered Nurse to join our care team as a Palliative Care Specialist. As a key member of our team, you'll evaluate patients, create personalized care plans, and ensure effective communication among patients, families, and the care team.

Responsibilities

  • Evaluate patients and develop comprehensive care plans that meet their unique needs
  • Communicate effectively with patients, families, and the care team to ensure seamless care coordination
  • Collaborate with interdisciplinary teams to deliver high-quality, patient-centered care
  • Participate in ongoing education and professional development opportunities to enhance your skills and knowledge

Requirements

  • Graduate of an accredited school of nursing with a current state license as a registered nurse
  • 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
  • Reliable transportation and ability to lift at least 80-100 lbs
