Urology Advanced Practice Provider

Community Health Partners is seeking a full-time Nurse Practitioner or Physician Assistant to join an established urology practice. The APP will collaborate with two urologists who specialize in treating a variety of urologic conditions. This is a great opportunity to become a contributing member of a team that prides itself in delivering high-quality care.
ResponsibilitiesThe Advanced Practice Provider (APP) will be responsible for providing clinical care in general or specialty areas. They will assess, plan, and provide patient care in collaboration with the attending physician and interdisciplinary healthcare team. The APP will perform comprehensive assessments, diagnose medical conditions, write prescriptions, and formulate treatment plans. They will also assist in basic urologic procedures and contribute to quality and regulatory activities.
Accountabilities- Provides direct assessment, evaluation, and primary patient care, complying with medical protocols and procedures.
- Participates in daily rounds, patient assessments, and care plan development.
- Orders, interprets, and evaluates diagnostic tests to identify and assess patient healthcare needs.
- Furnishes medications based on written protocols.
- Makes referrals to appropriate providers when patients require subspecialty medical attention.
- Documents and maintains precise and complete records of patients.
- Provides education, training, information, and consultation services to the interdisciplinary team, patients, family, and caregivers.
- Participates in quality assurance activities, including assistance in development and implementation of new policies and procedures.
Education: Completion of a Nurse Practitioner or Physician Assistant program required. Credentialed by the CMC Medical Staff Office as an Allied Health Professional and privileged to perform Nurse Practitioner or Physician Assistant Standardized Procedures required. Experience: 2 years of clinical experience in the area of specialty preferred. Licenses and Certifications: If qualifying as a Nurse Practitioner, the following is required: NP - Current State of California Nurse Practitioner License required, NPF - Nurse Practitioner Furnishing Number required. If qualifying as a Physician Assistant, the following is required: PA - State of California Physician Assistant License required, DEA - DEA Controlled Substance Number required, BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required, Ability to participate in a federally funded program (Medicare and Medicaid) required. NPC - National Certified Nurse Practitioner by ANCC (American Nurses Credentialing Center) or AANP (American Academy of Nurse Practitioners) preferred, CRNFA - Certified RN First Assistant preferred.
