LPN - Licensed Practical Nurse - FT - Days

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at St. Vincent's. As an LPN, you will provide high-quality nursing care to our residents, working collaboratively with our interdisciplinary care team to ensure the best possible outcomes.
Responsibilities- Provide direct patient care, including administering medications, performing treatments, and documenting resident interactions.
- Contribute to the assessment of residents, identifying their physical and psychosocial needs, and developing individualized care plans.
- Collaborate with healthcare professionals to ensure seamless care transitions and effective communication.
- Document resident interactions, outcomes, and medication administration, maintaining accurate and up-to-date records.
- Participate in ongoing education and training to enhance your skills and knowledge, staying current with best practices and industry developments.
- Graduate of an accredited practical nursing program with a current LPN license.
- Experience in long-term care or a related setting, with a strong understanding of gerontology and geriatric care.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Flexibility to work a variety of shifts, including days, evenings, and weekends, as needed.
