Medical Coordinator Specialist

As a Medical Coordinator Specialist with Resources for Human Development, you will play a vital role in ensuring that our program consumers receive high-quality healthcare services.
Job SummaryWe are seeking a licensed practical nurse to join our team. The successful candidate will work closely with our RN and site supervisors to coordinate healthcare services, maintain accurate documentation, and communicate effectively with other staff members.
Responsibilities- Maintain accurate nursing documentation and medication records, ensuring compliance with regulations and standards.
- Assist residents in learning about their medications and ensure they are taken as prescribed.
- Provide essential nursing care, including wound care, tracheostomy care, diabetic counseling, and more.
- Conduct assessments of residents' health and emotional needs, identifying areas for improvement.
- Communicate effectively with nursing and program staff, escalating serious issues to Nurse Manager and Program Director as necessary.
- Maintain confidentiality and adhere to RHD's values and principles.
- Accompany staff at medical appointments to educate on visit reasons and clarify orders as written by healthcare providers.
- Coordinate with hospital case management to share information and ensure continuity of care.
- Facilitate regular communication with hospital case management during admissions, maintaining contact and attending discharge planning meetings as needed.
- Participate in multi-disciplinary team meetings and case conferences to provide information on resident healthcare.
- Collaborate with Program Director/RN and consumers' teams to ensure documented information is available for Durable Health Power of Attorney/Advanced Directives.
- Schedule initial and follow-up appointments, completing corresponding appointment consult paperwork.
- Ensure accurate completion and currency of healthcare releases and consent forms for continuous care.
- Track and notify Program Director/RN of consumer refusal or challenges.
- Maintain Lifetime History records for all consumers, updating annually.
- Current licensure as a Licensed Practical Nurse (LPN).
