Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Wadsworth Glen Health Care and Rehabilitation Center in Middletown. As a valued member of our care team, you will have the opportunity to make a meaningful difference in the lives of our residents.

About the Role:

As a Licensed Practical Nurse, you will provide direct nursing care to residents under the supervision of the Registered Nurse (RN). Your responsibilities will include:

  • Assessing and addressing the nursing and rehabilitation needs of each resident
  • Maintaining accurate and up-to-date resident care plans
  • Administering medications and treatments as prescribed by physicians
  • Providing education and support to residents and their families
  • Collaborating with the interdisciplinary care team to ensure high-quality care
Requirements:

To be successful in this role, you will need:

  • A current, unencumbered license to practice as an LPN/LVN in this state
  • Graduation from an approved LPN/LVN program
  • CPR certification
  • A minimum of one year of experience in a hospital, long-term care facility, or other related healthcare setting
