Palliative Care Registered Nurse

Illumia Health, a part of Gentiva, excels in hospice and palliative care. Our clinical teams provide advanced illness management, offering relief from symptoms and stress of illness while improving quality of life. We are seeking a compassionate and skilled Palliative Care Registered Nurse to support patients and families.

Key Responsibilities:

  • Deliver preventive and supportive care to palliative patients and families, utilizing assessment, intervention, and teaching skills to maximize patient comfort and enhance quality of life
  • Collaborate with the patient/family, attending physician, and other members of the interdisciplinary team in providing patient and family care
  • Document the problems, appropriate goals, interventions, and patient/family response to palliative care
  • Effectively communicate with members of the team, clinical supervisor, and the patient's PCP regarding changes in patient status and/or patient needs
  • Maintain accurate and relevant clinical notes regarding the patient's condition

Requirements:

  • Associate's/Bachelor's degree in Nursing
  • 2 years of prior skilled Registered Nurse experience
  • Current RN license to practice in the state
  • CPR Certification
  • Competence in computer programs Microsoft Outlook, Word, and Excel
  • Prior clinical experience and strong compassionate phone skills
