Qualifications for an ideal Licensed Practical Nurse (LPN):

  • High school diploma or equivalent (GED)
  • Valid LPN License in the relevant state
  • First Aid & CPR Certification
  • Minimum of 1 year of relevant experience

Licensed Practical Nurse (LPN) Role Overview:

The Licensed Practical Nurse (LPN) will facilitate and coordinate the provision of care in partnership with physicians and care team members. Most importantly, you will demonstrate a commitment to serving seniors and fostering a positive atmosphere that our residents cherish.

  • Conduct assessments, develop plans, implement care, and evaluate resident outcomes.
  • Deliver exceptional care by performing regular rounds, ensuring seamless care transitions, documenting essential care information, and monitoring residents' overall health and behavior.
  • Assist the Director of Wellness in formulating and revising resident care plans.
  • Maintain continuous communication with residents, families, and healthcare providers regarding resident care.
