Job Description

About the Role: We are seeking a highly skilled and motivated Home Health Physical Therapist Assistant to join our team at All Medical Personnel. As a key member of our interdisciplinary team, you will be responsible for providing high-quality physical therapy services to patients in their homes.

Key Responsibilities:

  • Collaborate with the patient, family, caregiver, and primary care provider to develop a personalized plan of care focused on achieving the patient's physical therapy goals.
  • Provide skilled, intermittent physical therapy services to patients in their homes, utilizing evidence-based practice for assessment, clinical decision-making, and coordination of care.
  • Act as case manager and team lead for patients requiring home health therapy services, when nursing is either not involved in the case or physical therapy is the primary patient need.
  • Assess patients and collaborate with the interdisciplinary team and primary care provider to develop the patient-centered plan of care.
  • Provide physical therapy treatment to patients/family/caregiver as ordered, and complete accurate clinical documentation to effectively support service provided within specified timeframes.
  • Supervise Physical Therapy Assistants, Home Health Aides, and students in the clinical setting, ensuring they meet the highest standards of care.
  • Maintain open communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods.
  • Report acute medical problems and condition changes to the primary care provider.

Requirements:

  • 1-2 years of experience in an outpatient rehabilitation setting.
  • At least two (2) years of physical therapy experience, preferably in home health or other health settings with a geriatric emphasis.
  • Current license in Physical Therapy or ability to obtain prior to hire.
  • American Heart Association Healthcare Provider BLS preferred upon hire, and required within 90 days of hire.
  • Current Driver's license.
  • Excellent customer service skills and ability to work without direct supervision.
  • Knowledge of physical therapy assessment and treatment methods, modalities, and equipment.
  • Ability to maintain effective relationships with other healthcare professionals and develop skills in technology used in home care settings.
  • Demonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff members.
