About the Role

We are seeking a compassionate and dedicated individual to join our team as a Family Model Provider. As a Family Model Provider, you will have the opportunity to make a meaningful difference in the lives of individuals with disabilities by providing a safe and supportive home environment.

Key Responsibilities
  • Provide housing, supervision, and support to individuals with disabilities in a family home setting
  • Assist with daily living activities, such as bathing, personal hygiene, toileting, ambulation, dressing, eating, and meal preparation
  • Transport and attend medical appointments, community outings, and recreation
  • Administer medication (if certified) and provide other necessary support
Requirements
  • 1 year of caregiving experience (preferred)
  • Valid TN Driver's license with auto insurance
  • Pass a drug screen, background checks, and reference check
  • Provide or obtain TB screening, CPR & First Aid certification, and other required trainings
  • Pass a home inspection
  • Ability to read and write in English and have basic computer skills
