Local Contract Nurse RN Care Manager

We are seeking a skilled Care Manager RN to join our team at ProKatchers LLC. As a Local Contract Nurse RN, you will play a vital role in providing exceptional patient care and support to our clients.
Key Responsibilities- Review and Evaluate Assessments: Conduct thorough reviews of member assessments and UAS information to identify potential concerns and develop effective care plans.
- Develop Working Relationships: Establish strong relationships with Primary Care Physicians (PCPs) to discuss member care and identify opportunities for improvement.
- Identify Risk Factors: Assess member risk factors and assign risk categories to ensure timely interventions and prevent unnecessary hospitalizations.
- Develop Care Plans: Collaborate with the Care Management team to develop comprehensive care plans that address member needs and promote quality care.
- Monitor Member Conditions: Regularly monitor member conditions, typically through telephone calls, but also through face-to-face interactions when necessary.
- Identify Clinical Issues: Recognize clinical issues that require immediate attention and take steps to reduce the risk of unnecessary hospitalizations, ED visits, or nursing home admissions.
- Improve Quality of Care: Identify opportunities to enhance the quality of care by ensuring members receive necessary preventative and chronic disease care.
- Education and Experience: Five years of clinical experience as a Registered Nurse, with leadership experience preferred.
- Skills and Certifications: Computer literacy, experience with Care Coordination, Utilization Review, and Discharge Planning, and PRI and Screen certification within three months of employment.
- Licensure and Certification: Active New York State License as a Registered Nurse, Bachelor's Degree required, and Master's Degree and Bilingual Spanish Certification in Case Management preferred.
