Job Summary

We are seeking a skilled and compassionate Healthcare Intake Coordinator to join our team at United Surgical Partners. As a key member of our intake team, you will play a vital role in ensuring seamless transitions for patients and families navigating our home health and hospice services.

Key Responsibilities
  • Respond to incoming calls from referral sources and patients to assess their needs and determine the best course of action.
  • Record and track referral information, making follow-up calls as necessary to ensure a smooth transition to care.
  • Collaborate with clinical teams to deliver high-quality services and address any concerns or issues that arise.
  • Communicate effectively with patients, families, and referral sources to ensure clear understanding of services and expectations.
  • Identify potential payer sources and negotiate rates and authorizations as needed.
Requirements
  • High School Diploma or equivalent required.
  • Minimum three years of experience in healthcare delivery or related business field.
  • Strong knowledge of medical terminology and customer service principles.
  • Effective data entry and word processing skills, with ability to problem-solve and communicate effectively.
