Speech Language Pathologist Home Health Opportunity

Franciscan Health is seeking a skilled Speech Language Pathologist to join our Home Health team. As a Speech Language Pathologist, you will play a vital role in providing comprehensive speech and language evaluations, treatment, and related follow-up services to our patients.

Responsibilities:
  • Perform comprehensive speech and language evaluations, treatment, and related follow-up services.
  • Document patient assessments and pertinent information in a timely manner.
  • Complete appropriate goal re-assessment and discharge planning.
  • Provide speech therapy, including selection and use of assistive equipment.
  • Collaborate with patient and other healthcare personnel or outside agencies regarding coordination of care.
  • Provide speech and language therapy according to standards of care defined by the state practice act and other professional and governing agencies.
  • Train and educate patients and family members or others on treatment plan and goals.
Qualifications:
  • Master's Degree - Required
  • 1 Year Speech Therapy - Required
  • 1 Year Home Health - Preferred
  • Speech Pathologist (SPE) - Required
  • American Speech Language and Hearing Association (ASHA) - Required
  • Cardiopulmonary Resuscitation (CPR) - Required
  • Driver's License - Required
  • Auto Insurance - Required
