Key Responsibilities

As a Remote Health Navigator with CareHarmony, your key responsibilities will include:

  • Managing patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
  • Identifying and coordinating community resources with patients that would benefit their care.
  • Providing patient education and health literacy on the management of chronic conditions.
  • Performing medication management including identifying potential medication concerns, reconciliation, adherence, and coordinating refills.
  • Assisting in ensuring timely delivery of services to patients, including Home Health, DME, Home Infusion, and other critical needs.
  • Resolving patients' questions and creating an open dialogue to understand their needs.
  • Assisting/Managing referrals and appointment scheduling.

We are looking for someone with a strong ability to work independently, excellent communication skills, and a commitment to delivering high-quality patient care.

The estimated salary for this role is around $48,000 - $58,000 per annum, depending on experience. Benefits include health benefits, paid holidays, paid time off, and sick time off.
