Social Services Coordinator

We are seeking a dedicated and compassionate Social Services Coordinator to join our team at WestLake Care Community. As a key member of our community, you will play a vital role in promoting and enhancing relationships between residents, their families, and the support systems and resources within and outside our community.
Key Responsibilities:
- Advocacy and Support: Develop one-to-one relationships with residents and families as needed for support, refer residents to social, health, mental health, and community agencies, and ensure documentation is in the resident's chart regarding the results of referrals.
- Empowerment: Assist residents to find meaningful work and a sense of purpose in the community, develop the social care component of the Care Plan for each resident, and ensure resident and staff participation in their care plan.
- Mediation: Support residents and families in dealing with feelings about death or dying and other emotional, mental, environmental, or physical limitations, provide resources inside and outside of the community, and work knowledge of advanced directives and palliative care.
- Safety: Be familiar with disaster planning policies and procedures, promptly report any occurrences or potential abuse immediately, and comply with HIPAA and Confidentiality policy at all times.
Requirements:
- A strong desire and passion to work with people who are unwell, disabled, or living with dementia.
- Composure: Cool under pressure, does not become irritated or defensive when times are tough.
- Conflict Management: Views conflict as an opportunity; settles disputes equitably.
- Listening: Practices attentive and active listening; has the patience to hear people out.
- Negotiating: Can negotiate in tough situations; gains trust and has a good sense of timing.
- Priority Setting: Spends time on what's important, eliminates roadblocks, and creates focus.
- Bachelor's degree in Human Services field strongly desired.
- A basic understanding of how to support those living with mental health, alcohol/drug addiction, post-traumatic injuries, physical disabilities, and dementia.
