About the Role

We are seeking a compassionate and dedicated Registered Nurse (RN) to serve as a Swing Bed Liaison at Deaconess Illinois. This role will play a key part in the growth and development of our Swing Bed program, focusing on strengthening and expanding referral patterns with outside facilities and providers.

Key Responsibilities
  • Welcome and complete readmission education packets for patients
  • Round on inpatient and outside referrals to ensure seamless transitions
  • Meet with Emergency Department patients and families to educate them on our Swing Bed program
  • Participate in the Swing Bed Quality Assurance and Performance Improvement (QAPI) program
  • Schedule and lead care plan meetings
  • Update patient charts and attend weekly interdisciplinary team rounds
  • Contribute to discharge planning and community education initiatives
  • Collaborate closely with the Case Management team and fill in for case management in the event of an absence
Requirements
  • Active RN licensure in Illinois or a compact licensure state
  • BLS certification
