Job Summary

The Home Health Physical Therapist is a key member of our interdisciplinary team, responsible for evaluating, planning, directing, and administering physical therapy treatments to patients in their homes. This role requires a strong clinical background, excellent communication skills, and the ability to work independently.

Responsibilities
  • Evaluate patients' physical abilities and develop personalized treatment plans to improve their mobility and function.
  • Administer physical therapy treatments, including exercises, stretches, and modalities, to restore patients' physical function and prevent disability.
  • Delegate tasks and supervise assistants to ensure efficient and effective care delivery.
  • Collaborate with other healthcare professionals to ensure comprehensive care and achieve optimal patient outcomes.
  • Document patient progress and maintain accurate records to ensure continuity of care.
Requirements
  • Current Physical Therapy License in the state of practice.
  • 2 years of experience in physical therapy, preferably in home health or a similar setting.
  • Ability to work independently and make sound clinical decisions.
  • Effective verbal and written communication skills, with the ability to work with patients, families, and healthcare teams.
  • Valid Driver's License with proof of current insurance and reliable transportation.
  • Valid AZ Fingerprint Clearance Card and Current CPR Certification.
