Job SummaryWe are seeking a highly skilled and motivated Licensed Practical Nurse (LPN) to join our clinical team at Chapel Manor in Philadelphia, PA. As an LPN, you will play a vital role in delivering efficient and effective nursing care, achieving positive clinical outcomes, and ensuring patient/family satisfaction.
Key Responsibilities- Deliver high-quality nursing care to patients, including evaluating their condition, administering medications, and performing treatments as ordered by physicians.
- Contribute to nursing assessments and care planning, ensuring that patients receive comprehensive and individualized care.
- Supervise and monitor patient care provided by unlicensed staff, ensuring that care is delivered in a safe and effective manner.
- Collaborate with interdisciplinary teams, including physicians, social workers, dietitians, and other healthcare professionals, to achieve optimal patient outcomes.
- Graduate of an approved school for practical or vocational nursing.
- Currently licensed by the State Board of Nursing or pending licensed practical nurse licensure.
- Experience in rehabilitative or geriatric nursing preferred.
- Maintain current BLS/CPR certification, if employed at a skilled nursing center, or as required by the state of practice if an assisted living facility.
- A competitive hourly rate of $30.00 - $33.00 per hour.
