Job Overview

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our HouseCalls team. As a key member of our team, you will be responsible for conducting in-home assessments, performing evidence-based practice screenings, and communicating findings with our patients' primary care physicians.

Key Responsibilities
  • Conduct comprehensive physical exams, evaluate past medical history, and review vital signs.
  • Perform point-of-care testing (as appropriate) and identify diagnoses for active medical management.
  • Communicate findings with primary care physicians to inform potential gaps in care.
  • Identify urgent and emergent situations, intervene accordingly, and provide proactive care management.
Requirements

To succeed in this role, you must have:

  • A current license to practice as a Nurse Practitioner (NP) or Physician Assistant (PA-C).
  • Active national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification.
  • The ability to obtain national certification and/or NP or PA-C licensure in your assigned state by start date.
  • Excellent communication and interpersonal skills, with fluency in Cantonese.
Work Environment

This role requires flexibility and adaptability, with a 20-40 hour workweek available. You will be required to travel throughout your assigned region and beyond up to 5% of the time. New York residents can expect an annual salary range of $88,000 to $173,200, based on experience and qualifications.
