Job Overview

We are seeking a skilled Mental Health Advocate Coordinator to join our team at Pickett Fences Family Stabilization. This role will play a crucial part in empowering individuals and families to overcome obstacles and achieve their full potential.

Key Responsibilities:

  • Conduct comprehensive assessments to identify the unique needs and strengths of clients and their families.
  • Develop individualized treatment plans in collaboration with clients, families, and interdisciplinary team members.
  • Provide ongoing case management services, including advocacy, coordination of care, and referrals to community resources.
  • Monitor client progress and adjust treatment plans as needed to ensure optimal outcomes.
  • Serve as a supportive and empowering resource for clients and their families, promoting resilience and self-advocacy.
  • Collaborate closely with other healthcare professionals, including psychiatrists, therapists, and social workers, to provide holistic care.

Requirements:

  • Bachelor's or Master's degree in Social Work, Counseling, Psychology, or related field.
  • Previous experience in case management or behavioral health preferred.
  • Strong communication, organizational, and problem-solving skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Passion for making a positive difference in the lives of others.

Salary Estimate: $45,000 - $60,000 per year, depending on experience.
