Case Manager Specialist

The April Parker Foundation seeks a highly skilled Case Manager II to join our team. As a key member of our organization, you will play a critical role in identifying and assisting individuals with accessing culturally affirming social support services.
This is an excellent opportunity for a motivated professional to make a meaningful impact in the lives of our clients. We offer a competitive salary range of $23,931 - $29,439 per annum, plus a bilingual differential of $4,764 per annum.
About the Role:
- Conduct outreach events and activities to engage with hard-to-reach BIPOC Youth and Young Adult residents.
- Perform client intakes by phone, zoom, or in person to gather information about clients through assessments and screenings.
- Make adequate referrals and/or linkages to ensure clients obtain necessary resources.
- Communicate with external organizations to provide additional information and coordination to support clients.
- Assist clients with obtaining and completing forms, and understand and work with various support systems available in the community.
- Identify emergency needs and make approximate referrals to support clients in crisis.
- Serve as a primary advocate and peer support person to clients.
- Maintain confidentiality of records relating to clients' records.
- Prepare and maintain notes in the Unite Us Platform and produce monthly reports.
- Attend weekly/monthly staff meetings and participate in case conferences as needed.
Requirements:
- Bachelor's degree in psychology, counseling, rehab counseling, or a closely related field.
- A minimum of two years of related experience OR a Master's degree in social work, psychology, counseling, vocational rehabilitation, or a closely related field.
- Ability to understand the characteristics of a wide range of intellectual disabilities.
- Personal and professional experience with mindfulness or meditation work preferred.
- Possesses excellent interpersonal skills and the ability to handle emotionally charged situations.
- Ability to effectively communicate with external community partners, and clients verbally and in written correspondence.
- Technical proficiency with the Unite Us platform (Training Provided).
