Key Responsibilities
  • Evaluate patient conditions and develop personalized treatment plans to promote optimal recovery and independence.
  • Collaborate with nursing staff and patient families to implement follow-through programs that build on patient progress.
  • Communicate patient progress and concerns to supervisors and team members, ensuring seamless care coordination.
  • Document patient care in accordance with regulatory, licensing, and accrediting requirements.
  • Instruct patient families or nursing staff on follow-through programs to support ongoing rehabilitation.
  • Maintain a safe and clean work environment, adhering to company standards and regulatory guidelines.
  • Perform other duties as assigned, in accordance with the Company's Code of Business Conduct and applicable laws.
Requirements
  • Graduate of an accredited Physical Therapy program.
  • Current and unrestricted Physical Therapy license in the state where services are rendered.
  • Current CPR certification.
  • Strong organizational and communication skills.
