Position Overview
As a Hospice Nursing Coordinator, you will play a vital role in our organization. In this position, you will provide direct patient care while collaborating with an interdisciplinary team to ensure that patients and their families receive the highest quality of care.
Key Responsibilities
In this role, your responsibilities will include:
- Conducting initial and ongoing thorough assessments of patients and their families.
- Developing and updating patient care plans as necessary.
- Utilizing health assessment data to establish nursing diagnoses.
- Administering medications and treatments as prescribed by the attending physician.
- Providing guidance to patients and families to address nursing and related needs.
- Maintaining accurate clinical documentation.
- Engaging in communication with physicians and participating in interdisciplinary meetings to address the medical and emotional needs of patients and their caregivers.
- Ensuring adherence to all applicable state and federal regulations.
- Performing additional duties as assigned.
Qualifications
To be considered for this position, candidates should possess:
- A degree from an accredited nursing program and a current nursing license in the state of practice.
- A minimum of one year of nursing experience; three years is preferred.
- Experience in hospice care or a similar healthcare setting is preferred.
- A solid understanding of the hospice philosophy of care.
- A commitment to excellence in clinical practice and documentation.
