Job Summary

The ideal candidate for this role is a detail-oriented, organized, and self-motivated Licensed Practical Nurse/RN who is passionate about providing high-quality care to residents. As a member of our team, you will be responsible for collecting information to complete the Minimum Data Set (MDS) using the medical record, bedside assessment, and staff, resident, and/or family interviews.

You will participate in daily PPS meetings, weekly Medicare meetings, and monthly end meetings to ensure federal billing requirements are met. You will also review the Plan of Care at least quarterly and with each Comprehensive Assessment to ensure changes during the quarter are included and updated.

Requirements

To be successful in this role, you will need to have:

  • Licensed Practical/Vocational Nurse with current state licensure
  • Minimum three (3) years of clinical experience in a healthcare setting, with long-term care experience preferred
  • Minimum one (1) year of MDS experience
