Job Summary:

As a Medical Social Worker at PruittHealth Home Health, you will provide medical social services to patients and their families in their place of residence. Your role will involve assessing social service needs, developing care plans, and providing counseling to patients and caregivers. You will also participate in the agency's Performance Improvement Program and maintain accurate clinical records.

Key Responsibilities:
  • Assess patients' social service needs and develop care plans in consultation with the RN Care Manager and Clinical Manager.
  • Provide counseling to patients and caregivers to assist with coping with significant changes in patients' conditions.
  • Locate and obtain resources necessary for patients' financial, social, medical, and emotional well-being.
  • Maintain accurate clinical records to meet federal, state, and agency regulations and guidelines.
  • Participate in the agency's Performance Improvement Program and staff development programs.

Requirements:
  • Masters degree in social work from an accredited school of social work.
  • One year of experience in medical social work.
  • Licensure by the state (for NC and SC agencies).
  • Valid drivers license and automobile liability insurance.
  • Current CPR certification.
