Medical Unit Nurse Specialist

We are seeking a highly skilled and compassionate Registered Professional Nurse to join our Float Pool team at Albany Medical Center. As a key member of our healthcare team, you will provide exceptional patient care and contribute to the delivery of high-quality, patient-centered services.
Responsibilities- Assess, coordinate, plan, direct, implement, and manage the needs of assigned patients throughout the episode of care, with a focus on progress toward discharge.
- Coordinate patient care provided by the caregiving team, ensuring uniform delivery of care to customers, including referring facilities, hospital staff, and patients in inpatient, ambulatory, and community settings.
- Participate in the Nursing Professional Advancement Pathway (NPAP) and actively engage in performance and quality improvement activities to enhance customer/patient satisfaction.
- Initiate open communication, convey positive intent, and offer assistance to patients, families, and colleagues.
- Collect pertinent data relative to the patient's health or situation and integrate knowledge of evidence-based assessment parameters in care area.
- Prioritize objective and subjective assessment data, diagnose patient problems, formulate expected outcomes, and determine strategies for meeting individualized patient needs.
- Implement strategies to address health promotion, prevention of illness/injury, facilitation of healing, alleviation of pain/suffering, and supportive care, performing/delegating interventions according to the Nurse Practice Act and AMC policy/procedure.
- Coordinate care with the patient, family, interprofessional team, and community-based resources to affect safe transitions in accordance with federal and state regulations.
- Deliver patient/family education with respect to culture, values, developmental level, and readiness for learning, using ongoing assessment data to revise diagnoses, outcomes, plan, and implementation strategies.
- Collaborate with consumers of healthcare and other caregivers in the conduct of care, serving as a charge nurse for the unit/department and leading other caregivers within the professional practice setting.
- Seek knowledge and competence that reflects current nursing practice and promotes future thinking, contributing to quality nursing practice through participation in unit/hospital committees and shared governance.
- Maintain strict adherence to the Albany Medical Center Confidentiality policy.
- Incorporate Albany Medical Center Behavioral Standards and Guiding Principles into daily activities.
- Comply with all Albany Medical Center Policies.
- Demonstrate an understanding of the job description, performance expectations, and competency assessment.
- Participate in departmental and/or interdepartmental quality improvement activities.
