About the Position

Pinnacle Home Care Inc. is seeking a Full Time Physical Therapist to join our team in Bradford and Union County.

We strive to provide high-quality care to our patients, and as a Physical Therapist at Pinnacle Home Care Inc., you will play a vital role in delivering exceptional care in the comfort of patients' homes.

Job Responsibilities
  • Assess Patient Needs: Conduct thorough initial and ongoing assessments of patients, including OASIS documentation, to develop tailored treatment plans that meet regulatory and clinical standards.
  • Develop Treatment Plans: Collaborate with physicians and nursing supervisors to create treatment plans that cater to individual patient needs, adjusting as necessary.
  • Interdisciplinary Collaboration: Work closely with the interdisciplinary care team to optimize patient outcomes and ensure seamless care coordination.
  • Patient Education and Support: Provide education and support to patients and their families on therapy programs, empowering them to take an active role in their recovery.
  • Leadership and Communication: Supervise Physical Therapist Assistants every 30 days and communicate patient progress and changes with physicians and care teams.
Requirements
  • A graduate degree from an accredited Physical Therapy program.
  • An active PT license in Florida.
  • Current BLS certification.
  • A valid driver's license and reliable, insured vehicle for travel to patient locations.
  • A minimum of one year of experience as a Physical Therapist, preferably in home health.
  • Proficiency in electronic medical records is beneficial.
  • Physical capability to perform required tasks, including standing, bending, and lifting.
Salary and Benefits

The estimated salary for this position is $85,000-$110,000 per annum, based on national averages and industry standards.
