Job Summary:

We are seeking a skilled RN to join our team as a Weekend Call Triage Specialist. As a member of our palliative care team, you will provide exceptional nursing care to patients and their families during non-business hours.

Key Responsibilities:

  • Triage patient calls with critical thinking skills to determine the need for home visits or phone/video communication.
  • Provide emotional support to caregivers and make required notifications during patient deaths.
  • Document all calls and patient interactions in the electronic medical record.
  • Collaborate with the interdisciplinary team to ensure quality and safe delivery of care.

Requirements:

  • Current licensure in good standing as a Registered Nurse in the State of Illinois.
  • 1-3 years of experience in hospice/home health care nursing.
  • Basic computer knowledge and proficiency in electronic medical records.
