Responsibilities- Coordinate and oversee all aspects of patients' home care, ensuring seamless transitions and optimal outcomes.
- Collaborate with patient care teams to develop and implement comprehensive care plans, tailored to each patient's unique needs.
- Educate patients and their families on condition management, treatment options, and self-care techniques.
- Perform thorough physical assessments, vital sign checks, and medication management.
- Administer injections, IV therapy, and tube feedings as prescribed.
- Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) to ensure high-quality care delivery.
- Regularly communicate patient progress to the clinical manager, physician, and care team.
- One year of clinical experience as a Registered Nurse (RN).
- Current RN license, specific to the state(s) assigned to work.
- Minimum six months of home health experience.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
- Able to assess patient status and identify requirements relative to age-specific needs.
- Familiarity with general computer use, including user names, password concepts, internet, email, and navigation of computer desktop.
