About the Role

Amedisys, Inc. is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Care Specialist. As a key member of our care team, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Responsibilities
  • Coordinate and oversee all aspects of patients' home care, ensuring seamless transitions and optimal outcomes.
  • Collaborate with patient care teams to develop and implement comprehensive care plans, tailored to each patient's unique needs.
  • Educate patients and their families on condition management, treatment options, and self-care techniques.
  • Perform thorough physical assessments, vital sign checks, and medication management.
  • Administer injections, IV therapy, and tube feedings as prescribed.
  • Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) to ensure high-quality care delivery.
  • Regularly communicate patient progress to the clinical manager, physician, and care team.
Requirements
  • One year of clinical experience as a Registered Nurse (RN).
  • Current RN license, specific to the state(s) assigned to work.
  • Minimum six months of home health experience.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
  • Able to assess patient status and identify requirements relative to age-specific needs.
  • Familiarity with general computer use, including user names, password concepts, internet, email, and navigation of computer desktop.
