Job Title: Per Diem Utilization Management Specialist

Finger Lakes Area Counseling and Recovery Agency (FLACRA) is a growing, not-for-profit behavioral health and substance abuse treatment provider with decades of experience and multiple innovative programs located throughout the Finger Lakes Region.

We are seeking a Per Diem Utilization Management Specialist to join our team. The successful candidate will be responsible for engaging and enrolling new clients in our Health Home Program, managing assigned caseloads, verifying client eligibility, and coordinating with healthcare providers and community agencies.

Key Responsibilities:
  1. Manage assigned caseload of Outreach Clients by properly meeting requirements for contact and enrollment.
  2. Verify and confirm a client's eligibility for the Health Home Program by confirming Medicaid coverage, diagnoses, and risk factors.
  3. Coordinate with FLACRA Providers and outside Community Agencies to gather needed documentation to qualify Clients into the Health Home Program.
  4. Complete all enrollment documentation for a Client properly on paper and in Netsmart to successfully transition the Chart to a Care Manager once enrolled.
  5. Document all efforts of enrollment into the Client's Chart within Netsmart.
  6. Build and use effective communications strategies among peers, medical staff, addiction and mental health providers, and other community agencies through the use of electronic assisted devices, including Telehealth and other interactive technology.
  7. Help improve, measure, monitor, and sustain quality outcomes that focus on clinical indicators/performance measures, patient satisfaction, and plan adherence.
  8. Participate actively in team meetings and supervision to enhance professional development.
  9. Maintain excellent record keeping.
  10. Use of personal vehicle for work and potential transportation of clients.
  11. On-Call duties as assigned for nights and weekends.
