Key Responsibilities:- Provide primary care in a concierge setting, collaborating with a nurse practitioner to deliver high-quality patient care.
- Work closely with an interdisciplinary team to develop creative solutions and allocate resources effectively.
- Practice across the entire continuum of care, including Day Center-based clinics, home visits, skilled nursing facilities, and hospital settings.
- Enjoy autonomy and flexibility, utilizing your skills as a well-trained generalist to make a meaningful impact.
- Act as the primary care physician for PACE participants in community and institutional settings.
- Provide routine care, episodic, and urgent care, including initial and semiannual evaluations.
- Establish collaborative practice with PACE nurse practitioner and develop care plans for PACE participants in collaboration with the interdisciplinary team.
- Communicate effectively with consulting physicians and integrate recommendations into plans of care.
- Prescribe in accordance with state and federal regulations and participate in on-call rotation.
