Thrive Skilled Pediatric Care LLC is seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to provide high-quality clinical home care to medically fragile children in San Antonio, TX.
Key Responsibilities:
- Provide skilled nursing care to pediatric patients in their homes, focusing on their physical, emotional, and developmental needs.
- Develop and implement individualized care plans in collaboration with the patient's healthcare team.
- Conduct thorough assessments, monitor patient progress, and adjust care plans as needed.
- Collaborate with families and caregivers to ensure seamless care transitions and address any concerns or questions.
- Participate in ongoing education and training to stay current with best practices and advancements in pediatric home care.
Requirements:
- RN/LVN with a current valid license to practice as issued by the State Board of Nursing.
- Current CPR certification.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Flexibility to work varied shifts, including evenings, weekends, and holidays.
