About the Position

We are seeking a skilled Travel Case Management RN to join our team at TravelNurseSource. This 13-week assignment will provide an opportunity to work with a reputable healthcare organization, Titan Medical Group, in a dynamic and challenging environment.

Job Summary

The successful candidate will be responsible for providing high-quality patient care and case management services to patients in Henderson, NC. This role will involve working closely with the healthcare team to develop and implement individualized care plans, ensuring optimal patient outcomes.

Responsibilities

  • Provide comprehensive case management services to patients with complex medical needs
  • Collaborate with healthcare providers to develop and implement individualized care plans
  • Conduct thorough assessments of patient needs and develop strategies to address them
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements

  • Current RN licensure in the state of North Carolina
  • Minimum 1 year of experience in case management or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in electronic medical records and other healthcare software
