Nurse Practitioner Assistant

CAPITAL HEALTH CARE NETWORK is currently seeking a skilled and compassionate Nurse Practitioner Assistant to join our team. As a member of our healthcare team, you will play a vital role in providing high-quality patient care under the supervision of a registered nurse.
The ideal candidate will possess excellent communication skills, be able to work collaboratively with others, and demonstrate a strong commitment to delivering exceptional patient outcomes. Responsibilities:
* Provide direct patient care as defined in State Nurse Practice Act.
* Implement plan of care initiated by the clinician.
* Provides accurate and timely documentation consistent with the plan of care.
* Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
* Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the clinician and/or clinical supervisor.
* Uses equipment and supplies effectively and efficiently.
Requirements:
* Graduate of an accredited practical nurse or vocational nursing program.
* Prefer one year nursing experience. Community/home health or medical/surgical experience is preferred.
* Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
* Possess and maintain CPR certification.
* Complies with accepted professional standards and practice.
* Demonstrates good verbal and written communication, and organization skills.
Salary: $55,000 - $70,000 per year.
